I am 28 years old. I have been masturbating since puberty, although the frequency has decreased over the last two years. I sometimes have sexual intercourse with my girlfriend, usually about once a month.
For some time now, I occasionally ejaculate sooner than I would like during intercourse. However, I generally do not have this problem while masturbating. I also feel that my penis is smaller and less firm when flaccid compared with other men, and sometimes my erection does not feel as firm as I would like.
I previously completed a three-month course of Ayurvedic oil and capsules, but I stopped because I did not notice much improvement. My girlfriend has not complained about premature ejaculation so far, but she still seems to need more intimacy and closeness after intercourse.
What should I do to improve these concerns? What lifestyle changes, exercises, or treatments may help me maintain stronger erections, improve sexual confidence, and have better control over ejaculation?

Your concerns involve several different issues—occasional early ejaculation, erection firmness, concern about penis size, and your partner’s need for intimacy. These should not automatically be considered a single problem of “low sexual power.”
First, occasional early ejaculation by itself does not necessarily mean that you have premature ejaculation. You have mentioned that this usually does not happen during masturbation and that intercourse with your partner occurs only about once a month. In such circumstances, several factors may contribute, including greater excitement after a long gap, novelty, sexual expectations, performance pressure, stress, or other physical and psychological factors.
If ejaculation occurs earlier than you want during most sexual encounters, you repeatedly find it difficult to control it, and it is causing distress to you or your partner, a personal evaluation by a qualified sexologist may be appropriate.
Concern About Erection and Penis Size
The size of the penis in the flaccid state can vary with temperature, stress, and physical condition. Therefore, comparing flaccid penis size with other men is not a reliable way to judge sexual function.
However, if you frequently have difficulty achieving adequate erection firmness or maintaining an erection, there can be several possible contributing factors, including inadequate sleep, stress, lifestyle factors, smoking, alcohol use, certain medications, metabolic conditions, or other physical factors. A specific cause cannot be diagnosed from this question alone.
Improving Ejaculatory Control
Instead of constantly thinking, “I must last longer,” try to become aware of your level of arousal and take a brief pause before the level of excitement becomes too high. Slow breathing and reducing unnecessary physical tension may also help.
If the problem continues frequently, sexological counseling or appropriate behavioral techniques may be useful. Counseling does not mean that the problem is “only psychological.” Sexual concerns can involve physical, psychological, lifestyle, and relationship-related factors at the same time.
Your Partner’s Need for Intimacy
The fact that your girlfriend wants more intimacy after intercourse does not necessarily mean that she is dissatisfied with your sexual performance. For many people, emotional closeness, communication, affection, and a sense of connection are also important parts of intimacy.
It may therefore be helpful to talk openly with each other about what she enjoys, what kind of closeness she wants, and what feels comfortable for both of you. Mutual consent, comfort, and respect should always be maintained.
“Sexual Power” and Physical Fitness
You do not need to depend on a single “sexual power” medicine to develop a stronger body or improve sexual function. Regular physical activity, adequate sleep, a balanced diet, maintaining a healthy weight, managing stress, avoiding smoking, and limiting excessive alcohol are more important foundations.
Activities such as regular walking, running, swimming, or other cardiovascular exercise, along with strength training, can support overall physical fitness.
About Ayurvedic Medicines
You mentioned that you previously used an Ayurvedic oil and capsules for three months but did not notice much benefit. Therefore, it would not be advisable to start another capsule, oil, or “sexual power” product on your own.
In Ayurveda, treatment is generally individualized based on factors such as diet, digestion, sleep, lifestyle, stress, and other symptoms. Therefore, recommending a specific Ayurvedic medicine or dosage without an individual assessment would not be appropriate.
When Should You Consult a Specialist?
If premature ejaculation occurs during most sexual encounters, you repeatedly have significant difficulty achieving or maintaining an erection, experience a major change in sexual desire, develop penile pain or another unusual change, or these concerns are causing significant distress or relationship difficulties, it would be appropriate to consult a qualified sexologist or medical professional.
There is no need to feel ashamed about discussing these concerns. Sexual health should not be judged only by penis size or by how long intercourse lasts. Erection quality, ejaculatory control, sexual confidence, mutual consent, communication, and emotional connection with a partner are all important parts of a healthy sexual relationship.
