Dear Doctor, I need guidance regarding my sexual health because I am getting married in about six months and I am very worried that my past habits may affect my married life. I have been masturbating regularly for around five years and I still masturbate. Because of this, I am afraid that I may have damaged my sexual strength or may not be able to perform properly after marriage.
My main concern is that I seem to ejaculate very quickly. During masturbation, I can usually last only around 30 seconds, and I am worried that the same thing may happen during intercourse and that I may not be able to satisfy my future wife.
I also experience significant pain in my legs, joints and back after masturbation. Sometimes, when I wake up in the morning, the pain in my legs is so severe that I have difficulty walking. I want to know whether this can actually be caused by masturbation or whether there may be another physical reason behind it.
Another concern is that my penis does not become as firm as I expect. I also notice some bending in the middle of the penis. Its length is approximately 4.10 inches, so I am worried about whether its size is normal and whether it could affect my future sexual relationship.
During masturbation, I also feel that a considerable amount of semen is released. I am concerned that losing this much semen may cause weakness or affect my future sexual ability.
Since I still have about six months before marriage, I would like to know what I should do from now onward. Should I stop masturbation completely? Can my sexual stamina and erection improve? What could be causing the leg and back pain, and should I have any medical tests or consultation before marriage?
It is understandable to feel anxious when marriage is approaching and you are worried about your sexual performance. The first important point is that masturbating for several years does not automatically destroy sexual strength or future married sexual life.
Some of the symptoms you describe may have nothing to do with masturbation. In particular, significant leg and back pain that makes walking difficult in the morning should not simply be attributed to masturbation.
About ejaculating within 30 seconds
Ejaculating in around 30 seconds during masturbation does not necessarily mean that intercourse after marriage will always last the same amount of time. Masturbation and sexual activity with a partner are different experiences.
A habit of masturbating very quickly, anxiety about sexual performance and repeatedly worrying about “how long I will last” can contribute to difficulty controlling ejaculation.
Rather than trying to prove your sexual ability by timing yourself, focus on reducing anxiety, avoiding excessive hurry and developing better awareness and control.
Leg, joint and back pain
Mild tiredness after sexual activity can occur, but severe leg or back pain that makes walking difficult is not something you should simply accept as a normal effect of masturbation.
Possible causes can include muscular or joint problems, nutritional deficiencies or other medical conditions. A proper medical evaluation may therefore be appropriate.
Do not assume that semen loss is the cause without an examination.
Curvature of the penis
A mild natural curve can occur in many men and does not necessarily indicate a disease.
However, if the curvature is new, becoming more pronounced, associated with pain during erection or likely to interfere with intercourse, it would be sensible to consult a urologist.
The exact cause cannot be determined without an examination.
About your penis size
You have mentioned a length of approximately 4.10 inches. Penis size alone does not determine sexual ability or whether you can have a satisfying sexual relationship.
Sexual satisfaction depends on many factors, including erection quality, arousal, ejaculation control, communication, emotional comfort and mutual understanding—not simply penis length.
Does releasing more semen cause weakness?
The amount of semen released can vary considerably between individuals and from one ejaculation to another.
A larger amount of semen does not mean that your body’s sexual strength is being permanently depleted. Semen production and overall physical or sexual strength are not the same thing.
Therefore, you should not judge your health simply by the amount of semen released during masturbation.
Ayurvedic perspective
Ayurveda looks at sexual and physical wellbeing in a broader manner, including digestion, sleep, mental stress, physical strength and Vata dosha, which is associated with movement and nervous-muscular functions.
However, because you are also experiencing significant leg and back pain, it would not be appropriate to simply assume that the problem is due to an imbalance of Vata and start medicines on your own.
If Ayurvedic medicine is considered, the appropriate medicine, dosage and duration should be decided only after understanding your individual condition.
What can you do now?
- Do not feel guilty or frightened because you masturbate.
- If you masturbate very frequently or very quickly, gradually work on reducing the habit rather than becoming anxious about it.
- Maintain regular and adequate sleep.
- Include regular physical activity and moderate exercise.
- Avoid sitting continuously for long periods.
- Eat a balanced, nutritious diet.
- Avoid smoking and recreational drugs.
- Do not repeatedly test your sexual performance before marriage.
- Avoid self-prescribing sexual-strength tablets, supplements or oils.
Your goal over the next six months should be overall physical health, emotional confidence and healthy sexual habits, rather than trying to achieve a particular number of minutes.
Should you consult a doctor before marriage?
Because you have described significant leg and back pain with difficulty walking in the morning, a personal medical assessment would be advisable.
Your erection concerns and penile curvature can also be assessed properly through an individual examination. Depending on the findings, a doctor may decide whether any further tests are necessary.
Seek medical attention sooner if you develop severe or persistent leg pain, swelling, numbness, weakness, difficulty walking, back pain spreading into the legs, or increasing penile curvature or penile pain.
You still have around six months before marriage, so there is no need to assume that your future sexual life will be unsuccessful. Use this time to improve your general health, sleep, stress management and confidence, and obtain appropriate medical guidance where needed.
Your consultation and your information will be kept completely confidential.
Mini Case References
Case 1:
A man in his 30s was worried before marriage that years of masturbation had damaged his sexual ability. He usually ejaculated quickly during masturbation and was anxious about satisfying his future partner. During consultation, it became clear that he was judging his sexual ability almost entirely by duration. He was advised that masturbation and partnered sexual activity can be different experiences. Attention was given to reducing rushing, managing performance anxiety and developing realistic expectations. Over time, he reported better confidence and less fear about his upcoming marriage.
Case 2:
A man in his 30s reported back and leg pain after masturbation and believed that semen loss was making his body weak. Instead of assuming that masturbation was the cause, his overall physical health, sleep, daily activity and other possible causes of pain were considered. He received appropriate guidance and was advised to seek medical assessment for persistent symptoms. With attention to the underlying issues, he reported improvement in his discomfort and less anxiety about his sexual health.
